    I, the undersigned property owner or authorized occupant, acknowledge that I have requested ReLeaf Services to move, reposition, or temporarily relocate my trampoline and/or related accessories for the purpose of performing lawn maintenance services.
    ​I understand and acknowledge the following:
    1.) Trampolines and their associated components, including frames, nets, anchors, springs, pads, and accessories, may be subject to wear, deterioration, hidden defects, or damage during normal use and movement.
    2.) ReLeaf Services is not responsible for pre-existing damage, defects, improper installation, loose components, or deterioration of the trampoline or its accessories.
    3.) I voluntarily request that ReLeaf Services move the trampoline as an accommodation and understand that there is an inherent risk of accidental damage when moving large recreational equipment.
    4.) To the fullest extent permitted by law, I agree to release, indemnify, and hold harmless ReLeaf Services, its owners, employees, and representatives from claims for damage to the trampoline or related equipment arising from the ordinary handling and movement of the trampoline during lawn maintenance services, except in cases of gross negligence or willful misconduct.
    5.) I understand that ReLeaf Services reserves the right to refuse to move the trampoline if it is determined that doing so would create an unreasonable safety risk or risk of property damage.
    6.) I acknowledge that moving the trampoline is not included in standard lawn maintenance services and is being performed at my request.
